Post Acute Care Team

Intermountain has developed a team approach to partnering with Skilled Nursing Facilities (SNF). This Post Acute Care Team (PACT) includes an Advanced Practice Provider (APP) and an Intermountain physician who collaborates with SNF staff to care for Intermountain patients who are receiving medical and rehabilitation services in the SNF.

Our Infusion Pharmacy team is available to provide these medications at home to reduce your need to seek daily treatment at a facility. We also provide nutritional formulas ordered by your physician to enhance your body’s dietary needs at home. We utilize nurses and the latest equipment to help you administer your own treatments which gives you greater schedule flexibility and makes care convenient.
